Luffy says to Sanji, “If you don’t like your surname, you should take Zoro’s. It would suit you.” He says it like he's only thinking that changing one's surname is just that, a change in name.
Ussop says something in agreement with what just left their captain's mouth. Sanji sputters as he says, “It doesn't- That's not- That's not how it works.”
Nami says something like she consciously decided he didn't just speak. “Roronoa Sanji sounds good.” She nods as she says it, as if it's that easy to her too.
As Sanji turns his head to see Zoro's reaction, he is shocked to hear what the swordsman has to say. “It does sound good. You should keep it.” He says it with a smirk, like he knows what this conversation is doing to Sanji. The cook thinks, he probably knows and is just messing with me. Which might be the truth, but not for the reasons Sanji thinks.

So when a storm blows Luffy’s hat into the sea and Luffy intends to risk either his health by going after it or his happiness by letting it go, Zoro does not hesitate and dives into the cold roaring sea himself.
(In which a hat is saved, Zoro gets hypothermia, and the rest of the crew tries their best to get it through his thick skull that their health and happiness are dependent on his health and happiness.)

“Tell you what,” Sanji says, already mentally juggling the late hour, calculating the prep he has to do for tomorrow, reviewing the available contents in the fridge that he could throw together a take-out bag for this kid, and – he hates that he does this, now, especially with what he came from -- he takes in the kid’s tattered looking hoodie and sweatpants, wondering what he can afford – when all that is thrown to a halt by the sound of the kid’s stomach growling.
It’s loud, and hard to ignore. The kid’s smile fades once again, and he drops his arms to his sides, hands going inside the sleeves of his hoodie and head bowing. It’s not the sound someone’s stomach makes on a daily basis – it’s the sound of Sanji’s childhood. The sound of someone who hasn’t had a good meal in a while.

Gender, be it primary or secondary, is not a thing Zoro has ever bothered much with; it's just one of those concepts that he couldn't care less about. It always seemed like such a stupid thing, an excuse for people to be assholes, an excuse for weakness and low self control. He doesn't make excuses for himself and he certainly doesn't tolerate them from others.
Unfortunately, the rest of the world seems a little too concerned with his secondary gender, a whole set of expectations placed upon him just because he's an omega.

Their legs are tangled and their torsos are aligned and their mouths are locked together with bruising closeness. Sanji tastes chapped lips and—oh Seas, are they moving? Sanji’s eyes, closed when he braced for impact, snap open.
He is nauseous, he is shaky, he wants to die.
Sanji pulls back, scrambling for purchase except that’s almost worse—Zoro is everywhere in bulky muscle and sword scabbards and warm breath that steams in the night air.
